How do I creating a form that I can fill in with text

I would like to be able to scan a form into my computer
and the in WORD (or some other program) be able to fill
in the blanks.  I am trying to fill out and insurance
form that I have to repeatedly fill out by hand many
times a month.  If anyone has and ideas it would sure
help me out.

The best solution for this is the forms filling tool of ABBYY Finereader 6,
Corporate Edition, and if you have a lot of forms to fill as you appear to
do, this will prove worth the money. If not, the alternative is the clunky
method of using the scanned form as a background image and building a
protected form using table over it to take your data.
