Can't place picture in custom-sized document

I've tried all the format options for my picture.  I want it to be along the
top and left margin of a 8.5" by 4.25" document, margins are .25".

When I insert it, it displays in the vertical middle of the document.  Each
time I try to move it up, it moves down by the vertical length of the
picture.

From the File menu, select Page Setup and go to the Layout tab and make sure
that the Vertical Alignment of the page is set to Top.
