Quotes are automatically replaced with accents over vowels

When I am typing in Word and I type an open quote it does
not appear.  If I type a vowel after the quote, the vowel
gets replaced with a letter with two dots over it( ä, ë,
ï, ö, or ü).  At the end of a word, if I type a close
quote, it will not appear unless I press the space bar.  
This problem has only recently appeared and it may be due
to an update I downloaded recently or someone has changed
a setting on my computer and I do not know how to change
it back.  I have looked in the äutocorrect" options and I
have even turned them all off but I cannot stop Word from
changing these quotes into accents.

This is presumably happening outside of Word too, as you seem to be using
the United States-International keyboard layout.

Go to Control Panel, Regional and Language Options.
Click on Languages, then Details

Click on add to add an appropriate layout such as US and remove US
International.

It is not really related to Word, it has to do with the keyboard layout you
have selected in Windows. If you want to use the layout you have then you
can look to http://microsoft.com/globaldev/reference/keyboards.aspx to see
if what you want to type is available; if not then that same site will let
you look at other keyboards to select the one that will work best for you.
