How did you delete the index entry?

I deleted the entry by highlighting the entry in the
document view mode and pressing delete. I also tried just
deleting the entry in the index by highlighting it and
pressing backspace. They both worked fine until I saved
the changes and when I reopened the document it had this
weird view. Just half the screen and page breaks showing
with dotted lines.

Okay, the index is a single field (if you press Alt+F9, you'll see { INDEX }
with some switches). You can delete index entries from it. You have to
delete the XE fields that are used to generate the index. They will be on
the pages indicated in the index and are displayed when you display Hidden
text.
