template with fill-in blanks in the header and footer

In our conversion from AmiPro to MSWord, we are having
problems with one template.  The user needs to enter data
that will carry over from one page to the next at the top
and bottom of the form.  The user may produce any number
of pages in one document.  The logical placement is in the
header and footer.  We cannot figure out how to put fill-
in blanks in the header and footer except by using the few
fields available on File/Properties/Summary.  If we use
those fields, we can't lock the template.  If we don't
lock the template, users cannot use the fields.  If we
lock the template, users cannot get into the header and
footer.  Further, the names of the fields don't match the
data we are requesting and there are not enough fields.  
We want to make this process as smooth and simple as we
can for the users.  Any ideas?
In AmiPro, starting the template opened some drop-down
lists of the fields for data entry.  I tried rigging that
in Word by making the first page of the template a data
entry place for the fields, but I am not getting the
fields to be available both for fill in on that page and
for display in the header and footer.

You can use cross references (REF fields) in the header to reproduce
information from the form fields. Check the calculate on exit check box in
the form field. Alternatively if this is not a form (and indications are
that a form would be too restricting) then a userform to gather the data is
