Word holding codes; text not reflecting styles

I have documents where in the TOC field I see formatting
of the TOC styles that does not reflect the actual styles
and I cannot update the styles; the same documents I
remove H1 formatting and all of the H1 formatted text will
loose it's numbering and indenting. These documents have
been handled quite a bit.

I think Word is holding codes in hard returns, page
breaks, or sections breaks. Am I barking up the wrong
tree? Any suggestions about what is going on, and maybe a
solution? Thanks!

Heading styles and TOC styles are independent. Could you describe a bit
better what you're seeing?
