What are some ways to format a Company policy and
technical manual with a table of contents, chapters and
index?

The world doesn't stop at the borders of the USA. A4 is standard in several

I don't use A4 myself, but the author of the article lives in Australia. As
Graham points out, the Word community is worldwide, and A4 is standard
virtually everywhere outside the US.

Take a look at (and download the Word version of) <URL:
http://www.mvps.org/word/FAQs/Customization/CreateATemplatePart2.htm>

It includes a strong template for a technical manual.
