I want to insert a heavily formatted Word document into another word document

I have a 35 page document.

In my appendix I want to include the survey that I
administered.  This survey was typed up in another word
document, and has a lot of formatting to it.  When I try
to copy and paste, it all gets screwed up, and I CANT fix
it like that (it's just straight impossible).

Is there a way I can just copy and paste that whole 3 page
survey onto three pages of my new word document? just
like, as a picture or ANYTHING?

I've tried paste special - microsfot word document, but it
just pastes page one!

Also, with a simple copy and paste, it never copies the
footnotes, I have to copy them seperately, which then
delinks then.  Which i can live with, but maybe there is a
way to just import the entire word document in CORRECTLY.

And no, insert...file - does not work either.  The
formatting gets completely messed up, and it's unfixable
(hours spent on it!!!).

I have the survey in PDF format too, if I can do anything
with that.
