You could use what Word calls an "online form." Check this in help. For more
about online forms, follow the links at <URL:

Chapman's series of articles. That would take care of the things the user
types in and the protecting the format.
However, the data from the database would probably be handled as a
mailmerge. You can't merge a protected form. So the merge would have to be
done first and then the form protected. This can be done manually using the
Forms toolbar. Simply tell the user to click on the lock symbol after the
merge is completed. It could be done autotmatically, too, but that would be
much more complex to set up.
