Insert Table of Contents adds 2 of a page

I have a user who has made adjustments to some pages in
her document and when she reinserted her Table of
Contents, it displayed one particular page twice. This
doesn't happen to any other pages she modified.  The
duplicate entry can be deleted manually but does anyone
know why the duplicate is being created or how to stop it
from happening?

We are running Windows 2000 SP4 and Office 2000 SP3.

Does she have a manual page break before the heading in question? If so,
tell her to instead format the heading as "Page break before."
