I am creating a template in Word 2003.  The table of contents has heading 1,
2, and 3 automatically generated as follows:

TOC 1 - no page numbering
TOC 2 - no page numbering
TOC 3 - dot leaders and page numbering

Is it possible to create one table of contents that page numbers only the
third TOC level?

Yes, you need an \n switch. See
