Inserting footnote created duplicate of footer

I am working with a relatively simple document but when I attempt to
insert a footnote in the document, it duplicates the footer and places
it beneath the paragraph containing the footnote superscript.  For
example:

Paragraph with footnote1

<duplicate footer>

---------
1Footnote

<actual footer>

Can't figure out what's causing it or how to fix it. Duplicate footer
appears on screen and in print.
