Outlining

I am about to complete a long document with outlining in
it for a large project for an important client.

The outlining has driven me crazy -- Word starts and
restarts the sequence on its own, and in some cases
assigns outline levels in places where I have not told it
to. Worse yet, the whole thing changes every time the
document opens and closes

What am I doing wrong, aside from using outlining in the
first place?

probably, your numbering is setup wrong, take a look at:
