Repeating table captions

How do I repeat a caption for a table that breaks over
several pages?

I know how to put the caption in a header row and make it
repeat, but this doesn't work with the 'insert caption'
feature (unless you want to include the caption twice at
the beginning of the table).

depends on what kind of work this is (and is still a style-question).
You can crossreference to the whole caption on the second page. Or you
might split the table up into two parts, hence have two captions.
