Only page numbers referenced in index

Word 2000 - Windows 2000 operating system.

The index was generated using RD field codes because the
manual is made up of several separate .doc files.  The
problem is that the index is showing page references
without accompanying text.  The page number format in each
Word file includes chapter numbers.  To add to this
puzzle, the page number refernces in the index include an
extraneous comma before the page number (example: ",11-6,
11-8, 11-10").

As a test, I have taken a few chapters and stripped every
XE tag and then regenerated an index for only those
chapters and the index still shows page number references
that include a comma before-hand even though the chapters
have no XE tags.

I check Microsoft's site on dealing with corrupt files and
it suggested saving the .doc file as a .html and then
converting back to .doc.  I did this to a couple of files
and regenerated an index but still achieved the same
results.  

I am in desperate need of help.  This is a major product
manual with a pending delivery date.

Here's a guess, since nobody has any good ideas...

If you are tracking changes, try accepting the changes and turning off
the feature.
