The repeated label is probably because of a missing <<Next Record>> field in
one of the labels in the main document. You need such a field before the
first merge field on all of the labels except the first one on the sheet.
The empty labels are most likely caused by your not having the merge fields
set up in the corresponding labels on the main document.

If this is a tablet PC, I hear there are problems with mailmerge to labels
that produce such anomalies?
