Address block does not line name and address up on envelopes

When I use the address block in mail merge for envelopes, the name is in the
proper location but the address and state information is on the left margin.  
Any suggestions?

I am using Office 2003 Pro.

Don't use the address block field. Use the individual fields that make up
the address and make sure they are in the frame attached to the Envelope
Address paragraph style. You can, if you wish use the envelope template
downloadable from my web site to create your envelope merge document.
