When I use the address envelope function only a few of the relevant fields
appear in the Word document. How do I either create a new template or edit
the one that exists so that it pulls the first and last name on the first
line; company on the second, address on the third, and town, state and zip on
the last.

Or for the template - see http://www.gmayor.com/Macrobutton.htm and the
sample templates available for download from the downloads page of my site.
