Not much to go on there so I don't really know where to start.  But,
starting from the beginning, dee the article "Creating a Mail Merge Data
Source" at:

and, the article "How to create a Mail Merge" at:

http://www.word.mvps.org/FAQs/MailMerge/CreateAMailMerge.htm
