inserting checkboxes from If..

When I mailmerge with a IF...then statement and try to nest another
IF..Then...statement and then based in the condition I try to insert either a
check box or blank box but it doesn't insert either of them.  I have been
successful with when I use just the If...Then by itself it will insert one of
these boxes based on the result of the formula.  
Does anyone have any suggestion on what I am doing wrong?

The obvious question, that cannot be answered from the information that you
supplied, is: if the necessary condition of the outer If...then...Else...
construction has been satisfied for the inner If...then...Else...
construction to be evaluated.
