Create a standard letter using data from MS Word document fields

I want to merge data from standard fields in the source document (a client
information form) to a standard template letter template in Word.  It would
be a one-off letter rather than a mass mail merge . Maybe a macro would do it?

A macro could do it, so could mail merge.  But more would need to be know
about the structure of the client information form to provide more specific
assistance.

The current client info form has fields for demographic and clinical
information like age, name, diagnosis, referring medical practitioner, etc.  
The form is in standard A4 layout Title, First Name, Lastname etc.  I wish to
update it to include clinical notes from patient consultations and my
treatment recommendations -  and to combine this info into a letter/report
for the referring doctor.  I can redesign the form to help with a solution.

Use the CD to re-install it.  However, it may not actually be deleted,
possibly you were used to starting it by using a shortcut icon on the
desktop and that is all that was deleted.  Have you looked under Start>All
Programs>Microsoft Office to try and start it that way?  Have you tried
double clicking on a Word Document file (one ending in .doc) in Windows
Explorer?
