mail merge missing the middle name field

I am trying to create a mailing list by merging from my
outlook address book.  When I try to insert my fields,
there is no middle name field for me to insert.  How can I
get the middle name to insert?

You can see the middle name field if you open Outlook, select the contacts
folder you want to use as the data source, and use Outlook Tools|Mail Merge
to initiate the merge. You will see a much more complete list of fields (and
you may also notice that one or two of them are different names from the
ones you may have been using to date).
