Merge data missing

I have created a form which has multiple fields that data
is inserted from a source file, (word document with a
table).  When I merge the information, it skips the first
names at the top of the next page.  The data is in the
source file when I look.  I do not see the missing data
either when I preview the merge.

If you mean that the second page of the /output/ is skipping data, you
probably one  { NEXT } (<<Next record>>) field too many, possibly one
located at the beginning of your mail merge main document.
