Form letter with prompts only?

I want to create a form letter with merge fields or ask fields or prompt
fields, (what ever you call them) that prompt for Name, Address, and several
other fields. I create these letters one-at-a-time. There is NO merge file
and I don't want or need to make a merge file. I just want to fill-in-the
blanks each time in response to the prompts. Can this be done?
