I have created a fax cover sheet template. Is there a way to select a name
form my outlook address book and have the name, and phone/fax numbers
automatically entered onto the template?

You can do this with macros - a method is shown a
http://www.gmayor.com/Macrobutton.htm If you run a version of the code from
an autonew macro in your template it can be arranged for the name and
numbers to be inserted automatically.
