Mail Merge Problems with Mailing Labels

I have a list of name and addresses in a Word file that I
want to use as a data source for mailing labels.  The
problem I have is that when I go through the Wizard the
first page of labels get lost. The file is set up as
follows:

Skips Address
Street Address
City ST 94441]

Next Address
Next Street Address
Next City ST 94441]

etc.
I am use a ]as a record separator and None for a field
separator. Any suggestions.

OK, no problem here using Word 2002 and either a data source in .doc or .txt
format, but I would expect you to lose the first /record/ if there was no
header row such as

fieldname]

However, at the moment I can't see why you would lose a whole page of
records. But is this Word 2002 or 2003? And what format/file extension does
the data source actually have?
