Mail merge using custom outlook fields

In Outlook 2002 I have modified my Contacts form to use
non-standard fields suchs as "gender" en created a couple
of custom field.

I would like do a mail merge in Word (2002 as well) using
these additional fields, but all I get from the mail merge
wizard is a standard set of Outlook fields. How can I my
own?
