How is an ODC made?

How is an ODC file made?  What are the steps and from what
program is it made?

ODC files are generated by Office applications. I've never
met anyone who has been 100% successful in writing one "by
hand"; unfortunately, Microsoft has never issued a set of
guidelines or any other documentation on how ODC files need
to be constructed.
