FillIn fields display by default when you create a new document based on a
template that contains them. They do not display without a macro when you
open an existing document that contains them. I confirm that a FillIn field
in a header doesn't display with a new document in Word 2003. I get the same
behavior in Word 2000. Generally, headers are not the place for fields that
seek user input.
If the StyleRef field won't do what you need, you may have to consider using
either an Ask field or a UserForm.

One recent, new factor with fields (not) updating are the new security
measures in the newer SP updates and newer versions of Word. See this KB
