Creating Documents with Prompting or Pop-Ups

I need to create a document for end users to be able to
fill in ONLY the requested/required information, while
keeping the integrity/security of the rest of the document
in tact.  

I've set up a template with 'fill-in' fields and 'ask'
fields, but do not know how to get to the final step, or
end result, with the ability to open the document as
someone might and seeing the prompting or pop-ups asking
required field information.

Any thoughts???  Help please!

I recommend you shift to a User Form.  See:
