Merge Excel information into a form in Word

I'm looking to have information from an Excel file put
into a form.  How do I do that?  I've tried in Mail Merge,
but I haven't had much luck.

What kind of "form"? Can you describe how you created it?

Also, which version of Word are we dealing with?
