Step #2 of Mail Merge Wizard problem

In Step #2 of the Mail Merge Wizard, when I
click "Select from Outlook Contacts" and then I
click "Choose Contacts Folder" and then I click OK with
the "Contacts" highlighted, the Mail Merge Recipients
list only includes the Last and First Names. Problem:
it does not include the company name, address 1, address
2, city , state, and postal code. How do I fix this
problem
