Converting labels to mail merge source

Is there an easy way to convert labels in Word to a mail
merge data source?  I have tried to convert table to text
and then convert text to table for 4 columns(Name,
address, city, state,zip).  The data I am converting
contains addresses and it splits the name of the city into
4 columns.  Any help would be appreciated.
