I want to create template that, when opened, prompts the
user for 5-6 different pieces of information, then inserts
each piece into the document in more than one place.
I had it working with the fill-in field, but could not
figure out how to place the information in more than one
location.
I tried using an ask field, but thus far have been unable
to figure out how to do it. When the template is opened,
I am not prompted to enter anything.
I am sure the solution is simple. Yet it eludes me. Any
help would be appreciated.

An ASK field assigns the entered text to a bookmark. The content of this
bookmark can be inserted with a REF field.
A FILLIN field doesn't assign text to a bookmark, but you could bookmark the
field itself and achive the same ends.
The best way to gather a set of information is to employ a userform. There
is plenty of information about creating userforms on the MVPS web site
linked from my sig.
