Has anyone mastered how to use form fields that alctually
give people the option to fill in a form by clicking on a
button or being able to choose from a drop down menu etc.  
I'm trying to develop an in-house questionnaire for work
that I need to be able to let people e-mail it back to
me.  Therefore I need to develop a questionnaire that can
be filled in on line and need to make it as simple as
possible ie drop down arrors to make relevant selection
from list.  Yes / No boxes which they can tick etc.

Any words of wisdom would be gratefully appreciated as I'm
supposed to have this finished soon.
