How can I use FILLIN fields with text boxes?!

Please help me if you can.

I am running Office 97 pro on Windows 2000 Pro.

I am trying to create a form using FillIn fields as
prompts but over a picture of an application form.
Therefore cannot just put field questions in areas on the
page as once the text is inserted it moves the answer to
the next question on and into the wrong place.

I have therefore put text boxes over all the appropriate
areas on my picture of the application form.

Then, I am putting Fillin fields in order, into each
different box. This is fine until, after saving it as a
template and trying to use the template to create a new
document, the Fillin field questions seem to come up in a
totally random order!!! i.e although they are in
descending order on the page, it asks the last question
1st, the 5th question 2nd etc, etc.

Why is this and what can I do about it?

A direct email would be GREATLY appreciated :)

Don't use text boxes for this - use a table. Better still, don't use fillin
fields, use form fields - in table cells of fixed size.
