Hello,

I have created a form using the form toolbar. I have
inserted several check boxes and would like to know how to
format them for online usage.  The have two problems as
followed:

1. I would like to have the person to select the yes or no
options

2. I have several options for the person to select from
and I would like for them to select all that applies.

If you could be of any help I would appreciate it. Thanks
for the advice.

I'm not sure I understand the problems. You just insert
checkboxes, yes? How is it not working?
