How do I mail merge check box

I have a main document with has some check boxes. These check boxes need to
be checked (unchecked) based on data source value. I would like to know as
how to do this.

Use an If...then...Else field that displays the appropriate checked or
unchecked symbol box depending upon the value of the field in the
datasource.
