How do I setup a merge template without a data source?

I want to setup a simple merge document with fill-in fields only.  I am
unable to do this as the merge wizard limits merges to only identified data
sources and will not complete the merge unless a data sources is identified
(I have no data -- I just want the merge feature to pause and prompt me to
fill in fields and customize a form letter).  The toolbar is also disabled.  
Is there any better coverage of this feature other than the word help module?
