Breakdown of "Calculate on Exit" function in protected documents with TOC

I got some website feedback last night concerning my tips page on
repeating data.  The user was having no problem applying the {REF}
field method on new documents but it wouldn't work on several of his
existing documents.  On further review the failures always occured in
documents containing a TOC.

I have reviewed some simple sample documents that he provided which are
easy to replicate.  In every case regardless if the TOCs are in a
protected or unprotected section the expected behaviour of the
"Calculate on Exit" feature fails.

REF fields are not updated.  Calculation formula fields are not
updated.  Nothing.

If I unprotect>delete the TOC field>reprotect then the expected
functionality of "calculate on exit" is restored.

I realize that on exit macros can be used as a work around, but does
anyone have an explanation or reference to a KB article explaining the
cause of this behavior?

Here is a twist.  If the REF or Formula field is located before the TOC
in the document then updates do occur with "calculate on exit."  It
appears the TOC blocks processing of all fields following it in the
document.

Thoughts or ideas on why this is so?

The answer provided via different route by Bill Coan is to lock the TOC
with CTRL+F11 prior to protecting the form.
