Field problems with Mail Merge

I haven't used mail merge since the ancient days when you inserted the field
codes by yourself.
I am having two problems creating a label merge
1) for some reason word (2002) changes my courtesy titles to 0 instead of Dr.
2) the address block is funky and when I chose one format for the listing
names "James Q. Randell and Cindy Q. Randell" it changes all the entries with
no spouse name "Cindy Q. Randell" to "Randell Family"

Can anyone figure out why

Don't bother with the Address Block, just insert the mergefields that you
want in the configuration that YOU want them.
