Merging images in document header

I am having a problem getting pictures included a document header via a
merge variable to display and print.
The text in the document template is as follows:
{ IncludePicture "{MERGEFIELD LOC_PICTURE }" }

I have included the same in the main body of the document also.

After merging the data and previewing the document the picture only
displays in the main body. The preview allows me to do a alt + F9 and
this shows me the details linking to the picture. In both cases this
displays:
{ IncludePicture "C:\\Temp\\Header_test.jpg" }
So it seems as if the merge is correctly working in terms of getting
the file locations in to the actual document in both places.

Does anyone have any ideas about what is different between including
the image in a header rather than the body of the text and what (if
anything) I need to do differently?

For information I am using Word 2003 SP2 but may also want to do this
on earlier versions of Word if it is possible to do so.

It should work if the merge document is a form letter merge and you merge to
a new document then update all the fields in that document. Because of the
header you will need to use a macro to update - see
