Every time I open word it has a PDF 5.0 tool bar showing,
when I remove it and close out of word, it appears when I
open word back up.  I have even removed it and did a reset
on the customize toolbar window, but it is comes back when
I open word again.  How do I make it go away?

The *only* way to make it go away is to remove the add-in from the Office
startup folder - but then you'll lose much of the integration with Word. Why
don't you set your standard and formatting toolbars on two lines then park
it at the end of the standard toolbar - where it should stay.
