Cannot reset toolbar

Every time I open word it has a PDF 5.0 tool bar showing,
when I remove it and close out of word, it appears when I  
open word back up.  I have even removed it and did a reset
on the customize toolbar window, but it is comes back when
I open word again.  How do I make it go away?

Hi, Pam,

That toolbar is created by an add-in. Go to Tools > Templates and Add-Ins,
and look in the list of "Global templates and add-ins". Uncheck the one
whose name has something to do with PDF, and then close and restart Word. To
get rid of the toolbar permanently, click once on the template in the list,
note the file's location below the list, and then (after closing Word) move
or delete the file.
