A toolbar popped up on my "fax" sheet that says "send a
copy", "to", "cc", and "subject". I want to get rid of
it. I clicked on "customize", and the only toolbars that
I did not recognize were "envelope" and "menu bar". I
don't know if either of these are the one that I want to
delete, but I was unable to uncheck the box to the left
of the toolbar names.
Can you help?

This sounds like an email header. You don't say what version of Word you
have, but if it is a recent one, you need to click on the E-mail button on
your Standard toolbar, which is a toggle and will turn off the header.

The toolbar you see is toggled on/off in several ways. One is to choose
File - Send to - Mail Recipient. Another way is to click the Email tool in
the Standard toolbar.
