Ray,
I'm not familiar with that method of getting the CheckBoxFormField, but I
haven't worked with anything later than Word 2000 yet. However, the way
that I insert checkboxes, and this should work in later versions of Word, is
to display the Forms toolbar and click the specific button provided for
inserting checkboxes.

Chad,
Thanks for your assistance. It successfully added the
checkbox for me. Is there a way the user can just click on
the box and it automatically populate instead of bringing
up the "Check Box Form Field Option" menu? It's now given
me options to change the size of the box, select "checked"
or "unchecked", Run Macro....

In order to use formfields in a form, the document has to be Protected For
Forms. For more information, refer to the following which I lifted from a
Frequently Asked Questions message that is posted weekly in several of these

I've never seen these instructions but don't doubt that you found them. I
don't know what that "macro" does or where it resides. I tried your
directions in Word 2003 and they do produce a field that doesn't seem to do
anything.
Anyway, there are a number of ways to create and use clickable checkboxes in
documents / templates. Some require protected or locked forms, others do
not. Take a look at the Checkbox template available at
http://www.addbalance.com/word/download.htm#CheckboxAddIn for an exploration
of these.
