If you have removed the add-in and the icon is still present, then it has
been saved in normal.dot - you should be able to use Alt+drag to remove it
from the toolbar. If that doesn't work try double clicking the toolbar area
to open the customize window then drag it off. Failing that you'll have to
rename normal.dot to remove it.

...could also try resetting the toolbar before renaming Normal.dot.
