email toolbar appears automatically

I have one document which, when opened in Word 2003, automatically shows an
email toolbar with 5 lines of data (buttons, from, to, cc & subject).

How do I get rid of this toolbar - it disappears if I try and use the
toolbar customize option but comes back as soon as I close the customize
window.  Ideally I want to prevent it ever appearing automatically for any
document.

Also, for my information, what causes it to appear?  Is it something the
document author did or something Microsoft has programmed in?

File > send to > mail recipient will toggle the display of the extra
toolbars. Do so then save the document.

The E-mail button on the Standard toolbar is also a toggle.
