Thanks, Margaret. I thought the first part of the article (using the /a
switch) had solved the problem, but I now find it is something quite
different.
It is only happening in one particular document, one which has had track
changes on. The Header and Footer Toolbar (and the header space and footer
space) appear normally in the Original and Original with Markup views, but I
cannot get them to appear in the Final or Final with Markup views. I'm
sifting through the archives but, so far, nothing works.
I'll keep trying, but does anyone have any ideas non the amended problem?

I finally fixed the problem, but I still do not know what caused it. I
accepted all changes in the document and then the Header and Footer spaces
and Toolbar miraculously appeared again. It was fortunate that I did not
need to keep a record of all the changes, but if I had not wanted to accept
all changes, I wonder how the problem might have been solved then?

Disappearing header/footer boxes often signal a corrupt document. You might
want to port your content to a new document.
