My 'insert' drop-down menu is getting longer by the day, filled with blank
lines that occur initially after the 'Break' and 'Date/Time' bars.
Any ideas as to what is causing this and how to get rid of the blanks?

I suspect a poorly written Add-In.

See http://word.mvps.org/FAQs/Customization/CheckIfAddinsInstalled.htm for
information on finding out what Add-Ins have been installed and how to
manage them.
